LA Chiro Care, operated by Nareg Babayans, DC ("we," "us," or "our"), is required by law to maintain the privacy of your protected health information ("PHI") and to provide you with this Notice describing our legal duties and privacy practices. We are required to abide by the terms of this Notice as currently in effect.

How We May Use and Disclose Your Health Information

• Treatment — to provide, coordinate, or manage your chiropractic care, including consultation with other healthcare providers involved in your treatment.
• Payment — to bill and collect payment for services, including submitting claims to your health insurance or the insurer responsible for an auto-accident or personal-injury claim.
• Healthcare Operations — for quality assessment, staff training, licensing, and other activities necessary to run our practice.
• Appointment Reminders & Health-Related Communications — including reminders by phone, text, or email, and information about treatment alternatives or services that may interest you.
• As Required by Law — including public health, health oversight, judicial and administrative proceedings, and law enforcement purposes, when required.

Uses and disclosures not described in this Notice will be made only with your written authorization, which you may revoke at any time.

Your Rights Regarding Your Health Information

• Right to Access — you may inspect and obtain a copy of your health records, with limited exceptions.
• Right to Amend — you may request that we amend your health information if you believe it is incorrect or incomplete.
• Right to an Accounting of Disclosures — you may request a list of certain disclosures we have made of your PHI.
• Right to Request Restrictions — you may ask us to limit how we use or disclose your PHI for treatment, payment, or operations.
• Right to Request Confidential Communications — you may ask us to contact you in a specific way or at a specific location.
• Right to a Paper Copy — you may request a paper copy of this Notice at any time, even if you agreed to receive it electronically.
• Right to File a Complaint — if you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

How We Protect Your Information

We maintain physical, electronic, and procedural safeguards to protect your PHI in our records and on this website. Staff access to health information is limited to what is necessary to perform their job functions.

Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as any information we receive in the future. The current Notice will be available at our office and on this website.
